The admissions process can seem overwhelming to many families. We suggest you visit Belmont Manor for a tour to see our facility and an opportunity to ask questions. You can take a tour any day, including weekends. Please contact our Admissions Coordinator to schedule a tour.
After you choose Belmont Manor to provide care for your loved one, we will send a nurse to the hospital, other care facility, or the person’s home to do a screening. This is to obtain as much information as possible about the person’s medical condition.
We will review all the information to see if we are the appropriate place for either short-term rehabilitation or long-term care. Also, we will consider availability on the appropriate unit. If we determine that Belmont Manor can provide the care at the right time, we will contact you with the admission date and plan. At this time, you will let us know the name of the resident’s doctor. If the resident does not have one, we will help you select one from our list of credentialed physicians.
Once admitted, our nursing, rehab, dietary, and activities staff will do an evaluation and design a plan of care. This will include all medications, treatments, and rehab. We use Pharmerica as our pharmacy and will order all the medications. A short time later, there is some admission paperwork to be completed.
